Welcome to your secure business account setup. This comprehensive questionnaire collects essential information and documentation to verify your business, comply with federal regulations (including KYC, AML, and Beneficial Ownership rules), and establish your banking relationship.
 

What you'll need to prepare:

  • Government-issued photo ID for all signers and beneficial owners (25%+ ownership)

  • IRS-issued EIN Confirmation Letter (or SSN for sole proprietors)

  • Formation documents specific to your entity type

  • Proof of business address and DBA (if applicable)

  • Details on funding source and initial deposit
     

The form is divided into clear sections:

Business Information – Legal entity details, contact info, and initial funding
Section A – Universal requirements for all businesses (EIN/SSN, IDs, address proof, tax forms)
Section B – Entity-specific formation documents (LLC Operating Agreement, Corporate Bylaws, etc.)
Section C – Beneficial ownership & control persons (required under FinCEN rules) Section D – Authorized signers with specimen signatures and 2FA preferences
Section E – Final certifications and authorized signature
